NOTICE OF INTENT TO AFFILIATE and REGISTERED CENTRES

As a duly authorized representative of the Pregnancy Centre listed below, the undersigned hereby affirms that the Pregnancy Centre intends to affiliate with CAPSS /Care Net.  In consideration for the anticipated benefits of this dual affiliation, the Pregnancy Centre agrees to the following articles and thereby will AGREE TO become a “Registered Non- Affiliate Centre” with CAPSS at this time.

1.    The Pregnancy Centre agrees from this date forward to fully comply with each and every standard set forth in our Standards of Affiliation for pregnancy centres, attached hereto and incorporated herein by reference. Incorporation documents must be attached and this returned notice to affiliate.

2.    The Pregnancy Centre concurs with each and every requirement set forth in the Statement of
Principle attached hereto and incorporated herein by reference. The Pregnancy Centre agrees from this date forward to fully comply with such principles in all its activities.

3.    The Pregnancy Centre concurs with each and every affirmation set forth in the Statement of Faith attached hereto and incorporated herein by reference. Pregnancy Centre will assure that each board member, employee, and volunteer who serves the Pregnancy Centre is in agreement with each and every affirmation of this Statement of Faith.

4.    The fee for a Registered Non- Affiliate is $500 which is intended to assist with the administration and mentoring services CAPSS will be offering the developing Centre.

5.    At an appropriate stage in development, as may be determined by CAPSS, the Pregnancy Centre will complete a formal affiliation agreement with CAPSS/Care Net and will submit an application for the required on-site training and consultation.  

6.    Upon agreeing to these terms, the Pregnancy Centre may begin using the CAPSS / Care Net name in accordance with the same conditions imposed upon other affiliates.(Contact Care Net for name use requirements, logos and instructions for filing for d/b/a status)

7.    It is understood that CAPSS may determine at any time, in its sole discretion, to deny affiliation status to the Pregnancy Centre and to revoke the benefits of provisional affiliation granted hereby.

8.    Upon return of this signed agreement CAPSS will provide the centre with the following CareNet manuals Board Manual, Forms Manual, Policy and Procedure Manual, Fundraising Manual, Evangelism Manual and a Marketing Manual.

9.    Failure to affiliate within 24 months of this agreement, the Pregnancy Centre will be billed $465.00 for the 6 manuals received. The unaffiliated centre agrees to make payment for these manuals within 30 days of invoice.
